"Your Scan Is Normal" Is Not the Same as "Your Movement Is Normal"
Few phrases in modern medicine cause as much frustration as being told, "Your MRI is completely clean, so there's nothing wrong with you," while you still struggle to pick up a laundry basket, bend over to tie your shoes, climb stairs, or sit through a full workday.
Patients across McAllen, Edinburg, the Rio Grande Valley, and expanding throughout the South Texas San Antonio corridor frequently find themselves dismissed by high-volume assembly-line clinics after a normal X-ray or MRI reading.
Here is the diagnostic reality: An MRI or X-ray answers what a structure looks like while lying completely still. It cannot measure how a joint moves under dynamic force, how a nerve responds to movement, or how much load your muscles can tolerate in real life.
Episode 53: "The Scan Answered One Question. Not All of Them."
Comprehensive South Texas accident and rehabilitation standards recognize that normal imaging is valuable information—it rules out surgical emergencies, fractures, and tumors. However, normal imaging should be the start of a functional movement evaluation, never the end of the conversation.
"'Your scan is normal' is not the same as 'your movement is normal.' I see this with patients who were told nothing is wrong, but they still cannot lift laundry, climb stairs, sleep comfortably, or work without symptoms. Do not use a normal image to end a functional conversation... The scan answered one question. Not all of them. Good care does not stop at what the scan shows. It asks what the person still cannot do."
What Can a Functional Evaluation Reveal That an MRI Misses?
A static imaging scan provides a still photograph of anatomy. A multi-variable functional evaluation assesses the dynamic engine:
- Dynamic Joint Gliding & Segmental Restriction: A facet joint or costovertebral articulation can be severely restricted in movement without showing a structural fracture on X-ray.
- Neurodynamic Mechanosensitivity: A nerve root can be hypersensitive to stretch or sliding motions even if a static MRI shows no massive disc extrusion.
- Muscular Force Production Deficits: Scans do not measure quadriceps torque, deep cervical flexor endurance, or 360-degree intra-abdominal bracing capacity.
- PROMIS Physical Function Impact: Standard radiology reports ignore how pain affects your ability to carry groceries, drive along I-2 / I-35, or play with your children.
The Open Window Method: Decompression + Objective Functional PT
At South Texas Accident & Injury in Alamo, TX (serving the entire RGV and San Antonio patient corridor), we bridge the gap between normal scans and real-world recovery using The Open Window Method:
"Dr. Paul Sta. Juana, DC performs targeted Cox® Flexion-Distraction spinal decompression or gentle joint mobilization to unload stiff segments and reduce nervous system irritation. This opens a 'movement window.' Liza Sta. Juana, PT immediately uses this window to coach objective motor control, progressive load loading, and task-specific movement retraining."
TARGETED DECOMPRESSION → OPENS MOVEMENT WINDOW → IMMEDIATE OBJECTIVE FUNCTIONAL PHYSICAL THERAPY

Frequently Asked Questions
Why do I still have severe pain if my MRI or X-ray came back normal?
An MRI or X-ray takes a static picture of bones and soft tissues while you are lying down in a machine. It does not evaluate how your joints function when bearing weight, how your muscles stabilize your spine, or how your nerves tolerate movement. Severe pain can stem from joint mechanical restriction, nerve mechanosensitivity, or muscle weakness—none of which show up as a structural tear on a scan.
How does your South Texas accident and rehabilitation facility approach post-scan care?
Our central facility in Alamo, TX (serving patients across McAllen, Pharr, Edinburg, and the South Texas San Antonio corridor) uses scans as an initial safety check. We then perform dynamic physical evaluations—measuring range of motion, muscle strength, nerve gliding, and validated PROMIS Physical Function scores—to build a targeted DC+PT rehabilitation plan.
Can I receive care under an attorney LOP lien if my collision scan was normal?
Yes. Insurance companies frequently try to deny crash claims by pointing to "normal" X-rays. We provide personal injury attorneys with defense-grade objective functional documentation (NDI, ODI, and PROMIS metrics) that proves real physical impairment under $0 out-of-pocket Letter of Protection (LOP) liens.
